Basic information about gynaecomastia / correction of the male breast

Enlargement of the male breast can be caused by an excess accumulation of fat. On the other hand, it can also be caused by an excessive increase in mammary gland tissue, which can be congenital, the result of a hormonal disorder or due to taking certain medications. 

The imitation of a female breast shape is often a psychological burden for the affected men, so that activities in which they would have to show their upper body (e.g. visits to the sauna or swimming pool) are deliberately avoided.

Procedure for gynaecomastia surgery

Before any surgical treatment, the causes of gynaecomastia must be clarified to ensure that other, non-surgical treatment methods are not necessary. A hormone test of the blood and an examination by a urologist are useful so that pathological causes can be ruled out.

Enlargements on one side may indicate a tumour and require urgent treatment. A prior ultrasound examination of the breast may be necessary.

The aim of the operation is to reduce the size of the enlarged male breast by removing fat and glandular tissue, thereby restoring the natural contours of the male chest. In some cases, it may be advisable to tighten the skin around the nipple, for example.

With a pure fat accumulation can be a Liposuction to correct the problems by gently removing the fatty tissue through a 4 mm incision.

If there is also excess glandular tissue, this is removed through a fine skin incision at the lower edge of the areola. The nipple and areola remain intact. The glandular tissue is sent for histological examination to rule out a tumour, which is crucial for your safety.

Drains inserted during the operation are normally removed after 1-2 days. The suture material usually dissolves by itself and therefore does not need to be removed.

A compression waistcoat should be worn for six weeks after the operation to help the swelling to subside and to support skin tightening and contour formation. Any strenuous physical activity should be avoided during this period. However, the final result can only be seen after at least three months.

What is the difference between gynaecomastia and lipomastia?

Gynaecomastia and lipomastia are two different conditions that can both lead to an enlargement of the male breast, but they have different causes and characteristics. Alternatively, they are also referred to as true gynaecomastia and pseudogynaecomastia.

The Gynaecomastia is a benign enlargement of the mammary gland tissue in men. This enlargement can be caused by hormonal imbalances, such as an excess of oestrogen or a lack of testosterone. Gynaecomastia can occur at any age and is often associated with feelings of tightness in the breasts or increased sensitivity to touch. The breast tissue is palpable and feels firmer.

The Lipomastia (also known as pseudogynaecomastia) is an enlargement of the male breast caused by fat deposits without an increase in mammary gland tissue. This type of breast enlargement is often associated with obesity or a high body mass index and the tissue feels softer, similar to a woman's breast. Lipomastia can be influenced by a healthy diet and exercise.

What are the causes of gynaecomastia?

Gynaecomastia is a benign enlargement of the mammary gland tissue in men and can have a variety of causes. Common causes are

  • An altered ratio of testosterone and oestrogen
  • Chronic diseases such as renal insufficiency or liver disease as well as genetic disorders
  • Various medications, such as anabolic steroids, antidepressants, or drugs for the treatment of prostate cancer, as well as alcohol or drugs (e.g. heroin and marijuana) can trigger gynaecomastia
  • Increase in fatty tissue in the breast area (often associated with obesity)

What is pubertal gynaecomastia?

Puberty gynaecomastia describes a temporary enlargement of the mammary gland tissue in boys during puberty. It often occurs in approximately 60 % of boys, usually between the ages of 11 and 15, due to a hormonal imbalance. In most cases, no treatment is necessary as the hormonal imbalance corrects itself as part of physical development.
