Implant change - changing breast implants
Although renowned implant manufacturers now give a lifetime guarantee on their silicone implants, every body reacts differently to the implant. Over time, a tissue capsule may form around the implant, which contracts and causes pain (known as capsular fibrosis). However, the desire for a different implant size or shape can also be a reason for a change. We generally recommend having the implants checked after 10 years at the latest. We can then decide together whether a change is advisable.
Some patients no longer feel comfortable with silicone in their bodies, but still want fuller breasts. Here the Breast augmentation with autologous fat represent a sensible alternative.

How does a breast implant exchange work?
The operation is performed under general anaesthetic and takes approx. 2 hours depending on the technique and findings. Access is via the original incision to avoid additional scars. If capsular fibrosis has formed, the resulting tissue capsule and scar tissue are also removed. Once the old implants have been removed, the new implants can be inserted.
Depending on the size of the implant and the condition of the tissue, an additional breast lift may be necessary. However, we will discuss this with you in advance in a detailed consultation.
What needs to be considered after an implant change?
The wound drains inserted during the operation can usually be removed after 1-2 days and the suture material after 2 weeks.
For support during the healing phase, we recommend wearing a compression bra day and night for a period of 6 weeks. A special chest strap should also be worn for the first two weeks after the operation. You should also not sleep on your stomach for the first two weeks.
In general, you will be largely symptom-free again after just a few days and, depending on the activity, will be able to work after 1-2 weeks. If there are no complications, you should only resume sports training, lifting loads and overhead work after approx. 6 weeks.
The final result usually appears after 3-6 months. To help the scars heal, you can treat them with a moisturising cream. Also protect them from direct sunlight to avoid hyperpigmentation.
Questions and answers about changing implants
The pain can vary depending on the individual's sensitivity to pain and the type of technique used, but is usually very bearable and subsides quickly in the first few days after the operation. To alleviate the discomfort, we will provide you with painkillers and medication to take home to make you as comfortable as possible, minimise the risk of infection and promote rapid healing. The pain usually subsides after a week, but it can take several weeks or months for the swelling and bruising to disappear completely. The scars fade over time but remain permanently visible.
Even if a breast implant exchange is carried out in a qualified and frequent manner, specific consequences can occur in addition to general complications, which you should be informed about in advance.
So-called capsular fibrosis or capsular contracture can also occur after a replacement (see also Breast augmentation). In these cases, the breast implant and capsule should be removed. A new implant can then be inserted (implant exchange) or an alternative method of breast augmentation (e.g. with Autologous fat) can be used. The risk of capsular fibrosis can be minimised by a slightly rough surface of the implant, placement of the implant under the muscle and extensive protection of the tissue during the operation.
Another important aspect is the possibility of displacement of the breast implant (implant dislocation). Consistently wearing a customised bra with a breast strap after the operation helps to reduce the risk of this.
Even with newly inserted implants, we recommend having the implants checked by us after ten years at the latest, so that we can decide together whether it makes sense to replace the implants.
In addition, the breasts can continue to change over time due to ageing, pregnancy, breastfeeding, hormonal fluctuations or weight changes despite surgery, which can lead to sagging of the breast tissue and thus also of the implants.
As a rule, this is a self-pay service, so the costs are never covered by health insurance.

What should you look out for when choosing a doctor?
When choosing a specialist, make sure that you use the term Specialist in Plastic and Aesthetic Surgery. Other designations such as beauty doctor, cosmetic surgeon, etc. are unprotected and not a sign of qualification (more on "What should you consider when choosing a specialist?„.
